Access360
Access360 is a new program offered by St. Catherine’s Center for Children that works with individuals currently on Medicaid to improve their overall health and well-being. Our team works with clients to address the many social drivers of health that thus far have gone unmet. By providing assistance and connecting clients with essential services, we aim to improve their health and thus the health of the overall population.
Services Include:

Care Management
Our care managers facilitate communication and support clients and their families. By addressing multiple factors that influence health, management of care leads to better outcomes and improves client satisfaction. Our care management services also prevent unnecessary hospitalizations and can lead to reduced healthcare costs.
Nutrition Assistance
Our program connects clients with a number of nutrition-based services, including: food pantries, meal delivery, medically tailored meals, nutritional counseling and education, and more.


Tenancy Support
Homelessness and housing insecurity oftentimes play a major role in declining health in low-income and imporverished individuals. Through our program, we can provide temporary, emergency assistance to clients to help cover rent, security deposits, utilities, and other costs related to tenancy. This support can keep an individual from being evicted and put out on the street.
Transportation
One of the many reasons individuals, particularly those on Medicaid, do not receive necessary services is a lack of transportation. Whether they cannot afford a vehicle or live too far from public transportation routes, this continues to be a significant barrier for individuals who need services. Access360 can help provide clients with transportation to important medical and mental health appointments, job interviews, housing appointments, and more.

Eligibility
To be eligible for services, individuals must meet the following requirements:
- As this program specifically services those on Medicaid, those looking for services MUST be on Medicaid.
- This service is currently offered to residents in 8 counties specific to New York’s Capital Region. To be eligible you must live in one of the following counties: Albany, Columbia, Greene, Montgomery, Rensselaer, Saratoga, Schenectady, or Schoharie.
- Those seeking services must schedule a screening with a member of our team and meet at least one criteria from screening.
Screenings:
Screenings for admittance into Access360 do NOT need to be in person.
If you would like to have your screening in-person, our team will be happy to meet with you.
During the screening, a team member will ask the potential client a series of questions to determine eligibility.
